• Voluntary Service Animal Registration
  • Only for students with Certified Service Animals.
• ESA Request Provider Form
  • In addition to this packet, we must also have a note on your provider’s letterhead with the date of your most recent office visit, his/her professional credentials, and his/her signature.

• Emotional Support Animal – Request to Change Form
  • If you need to request a change to your currently approved emotional support animal, you will need to complete the above form.

• Housing Accommodations Provider Form
  • In addition to this form, we must also have a note on your provider’s letterhead with the date of your most recent office visit, his/her professional credentials, and his/her signature.
• Dining Request Provider Form
• Certiport – Excel Certification Exam
  • This is the portal for requesting accommodations on the Certiport Excel Certification Exam. Students can log-in once they have created a Certiport account but must request their documentation/access to their documentation by emailing disabilities@elon.edu. Students are now required to upload the documentation themselves.
